    Individual Provider Services
 

Solo Provider Startup Package  -       Starting at $1,250

Designed for providers establishing insurance participation for the first time.

Includes:

  • CAQH profile setup

  • Enrollment with up to 5 commercial payers

  • Payer-specific application preparation and submission

  • Payer follow-up and enrollment status tracking
     

Commercial Payer Enrollment  -          $175–$250 per payer

For providers enrolling with an additional commercial insurance payer.

Includes application preparation, submission, payer follow-up, and status tracking through determination.
 

Medicare Individual Enrollment — CMS-855I   -    $275

Complete individual Medicare enrollment through PECOS, including application preparation, submission, and routine follow-up.
 

Medicare Reassignment — CMS-855R  -        $150 per provider

Connects an individual Medicare-enrolled provider to an eligible group for billing and reassignment purposes.
 

Michigan Medicaid / CHAMPS Enrollment  -    $200

Michigan Medicaid provider enrollment and submission support, including routine follow-up through determination.
 

CAQH Profile Setup  -     $125

Initial CAQH profile completion, document upload, practice information, and preparation for attestation.
 
     Group Practice Services


New Group Practice Setup  -          Starting at $2,500

Designed for new group practices establishing their payer participation.

Includes:

  • Organizational enrollment

  • Group enrollment coordination

  • Type 2 NPI enrollment coordination

  • Up to 3 initial providers

  • Provider-to-group affiliation coordination

  • Payer submission and follow-up
     

Additional providers, payers, locations, and complex enrollment requirements may be quoted separately.
 

Medicare Group Enrollment — CMS-855B  -      $550

Medicare enrollment for clinics and group practices, including application preparation, submission, and routine follow-up.
 

Add Provider to Existing Group  -       Starting at $125 per payer

For an already-enrolled provider who needs to be added or affiliated with an existing group's payer participation.

Includes payer-specific affiliation/change submission, group information, applicable practice location updates, follow-up, and enrollment status tracking.
 

Updates & Changes


Provider Demographic / Practice Update  -      Starting at $75 per payer

Routine updates to payer records, including:

  • Practice or provider address changes

  • Contact information

  • Practice locations

  • Taxonomy updates

  • Other routine demographic changes
     

Complex Enrollment Changes    -    Custom Quote

Complex changes are quoted individually based on the scope and payer requirements.

Examples include:

  • Ownership changes

  • EIN/TIN changes

  • New practice locations requiring additional enrollment

  • Mergers or entity changes

  • Payer-specific corrections

  • Other complex enrollment or affiliation changes


Ongoing Credentialing Management - Credentialing Maintenance & Payer Management  - Starting at $100/provider/month

Ongoing management designed to keep your practice and providers current with participating payers.

Includes:

  • CAQH maintenance and attestations

  • Recredentialing tracking

  • Credential expiration monitoring

  • Payer demographic updates

  • Provider additions and terminations

  • Payer correspondence

  • Ongoing enrollment follow-up

  • Credentialing status tracking


    Please Note

    Payer fees, government fees, licensing fees, fingerprinting, background checks, and other third-party fees are not included in service fees unless specifically stated.  Pricing may vary based on provider specialty, number of payers, practice locations, ownership structure, and payer-specific requirements. Complex or multi-provider projects receive a customized quote before work begins.

Our Commitment
 

If an application submitted by Crane Provider Enrollment Solutions requires correction due to an error or omission on our part, we will correct and resubmit the application at no additional charge.
 

Additional fees may apply for changes requested after submission, incomplete client documentation, payer policy changes, or work outside the original scope of services.
